NHI frustrations still high; Some persons still cannot access cards

ROAD TOWN, Tortola, VI - While the crowd at the Joshua J. Smith Building was visibly smaller than it was at the end of last year, there are still a large number of dissatisfied persons who have expressed frustration at the process of waiting to be registered with the National Health Insurance (NHI) and waiting to collect their card.

One woman was peeved that she had to line up for a long time just to get her card, as this news site made a visit to the NHI office today January 5, 2016.

“I have to go to the doctor tomorrow and I need my card. I can’t go without my card,” she fumed.

One staff member said there was no dedicated employee dealing with the issuing of cards and that this had to be done by people also executing other duties.

“We are standing out here like beggars. You can issue the cards by checking our faces,” said one woman who was clearly frustrated with the long wait.

Another employee of NHI said the system experienced a breakdown and as a result some persons’ photographs got deleted from the system.

“All the photos taken got lost. The system was down. Persons had to be re-photographed. Some of the cards have not been printed as yet,” said the employee.

The NHI is sustainable

Minister for Health and Social Development Honourable Ronnie W. Skelton said the NHI has been studied and was found to be sustainable provided inherent risks are managed.

“Legitimate concerns were raised regarding the medium to long-term solvency of the NHI Fund,” said the minister through a Government issued press release.

“I am pleased to say that a new actuarial study has been completed and its projections are consistent with the NHI Financial Model Report prepared in 2012. Both reports showed the Fund to be sustainable, provided that its inherent risks are managed skilfully. I will shortly be seeking authorisation from the Cabinet to table these reports in the House of Assembly,” he said.

He noted that various risk management mechanisms are built in to the NHI design, including reinsurance to protect against catastrophic claims over Two Hundred and Fifty Thousand dollars ($250,000.00). “After evaluating three tenders, an agreement has been reached with Caribbean Insurers (Health) Limited to provide reinsurance coverage in affiliation with international partners with effect from January 1, 2016.”

He expressed gratitude to local insurance agencies for participating in the tender process, and for accommodating the needs of residents who wish to purchase supplemental health insurance policies. “Supplemental policies may be used to defray out-of-pocket expenses on co-payments and additional billing when annual limits have been exceeded; or in cases where the fees charged by private healthcare providers exceed NHI rates,” said the Minister.

Arrangements are also in place for residents to access specialised healthcare overseas on referral, or in cases of emergency when travelling abroad. An Agreement with the Florida-based JIPA Network and its affiliation with Aetna, Inc. and other international partners provides preferred access to hundreds of high quality medical facilities in the Caribbean, Latin America, and the United States. Through an affiliation with Europ Assistance, world-wide medical assistance will be provided in emergencies.

Review Committee scrapped

The Minister said that the NHI pre-authorisations for overseas transfers will be managed by an in-house Medical Director, in consultation with the patient's referring physician and overseas specialists. “The Chief Medical Officer, Dr. Irad Potter, will perform the duties of NHI Medical Director while recruitment continues for a full-time Medical Director. After consideration of public concerns and widespread misinformation, the decision was taken not to establish a Medical Review Committee,” he said.

“We will continue to engage with private insurance companies, healthcare providers and other key partners to monitor utilisation and make any further adjustments necessary to improve the NHI System as we move forward together,” said the Minister.

      They will be the first out of here to go receive medical treatments. I'm sure half of them won't go down Peebles unless it is absolutely necessary. And I will almost bet my right hand is taxpayers money paying their bills too. So them don't have not one thing to worry about in terms of NHI.

    That is what happens when people wait until the last minute.I signed up since October and have already used my zNHI card.
    I hope that they get a medical person soon.Dr Potter is not the one to have there.

      Depends on where you get treatment. If you can be treated at Peebles the NHI will pay 95%, if you have to go overseas the NHI will only pay 80%.

      There's no out of pocket maximum, so if you have to go overseas that's 20% of the bill out of your pocket with no cut off.

      Better hope you don't have a million dollar treatment plan...

        Proviso is up to NHI max. 20% of NHI Max and balance out of your pocket. Depending on what their max is that can be a catastrophic sum

      I called and asked the same question I was told to bring my son back home or have him access the care pay for it and seek reimbursement...........hmmm........ like I am going to bring him to use this health facilities who don't have specialists

        Seriously @Zumba I have private insurance with my employers so my child is covered........in regards to the previous poster College Students I am sure if they are a parent as well their child is also on their insurance so they are covered.....I am lucky that my employer is keeping our private insurance unlike others who isn't so for now my concerns regarding my child coverage is safe but we may never know what may happen later on.......

      Just because you went and got your done does not mean a thing. There are people registered from September, show photos got lost and was waiting for days to get completed and still dont have a card because they had to go thru the process again. People have lives and standing in line for days taking time from work is not easy for everyone. You comment is ignorant.

    NHI is good. I love it. I'm thankful for it. Only the staff can make it fail. Government has done a good thing.

    May God have mercy on he people of the BVI. The list of providers mentioned above are all blood suckers and would either bleed the last cent from the poor people or bankrupt the NHI. The Public Hospital does not have the necessary equipment that are needed to provide certain services such MRI and will never be provided by the government because if those services are provided by the Public Hospital the relatives and friends of the government will not be able to make money.
    My people of the BVI do not be fooled by this sugar coated promises of the minister of health. When you go to the Public Health service provider, that is the hospital and clinics it would be better to obtain your own of materials that are being used at the facilities because you are being billed for every piece of cotton, syringe and needle that are used.
    It is high time for the Health Service Authority get rid of pastor John Cline and use the salary to assist to defray the cost of the hospital services.

    Been there three times, stand in line for hours and still can't get my card. Card has to be redone. My God still did not get it done yesterday. Registered since September. Some workers handpick who they assist in a timely manner. System not properly organized.

    What do people who already have pre-exiting issues. Can we still go to St. Thomas, P.R. and the Mainland to continue our treatment that cannot be treated in the BVI. Can someone explain all this to me. I still have treatment to complete. Minister Skelton can I hear about that please.

      Have you spoken with your doctor here? Is your doctor's firm listed above? He/she is supposed to be in a position to let you know how your matter will be treated once NHI is in effect. This is a conversation that you and that doctor should have had months ago.

      @ people with issues; all I will say don't give up your private insurance as yet; I will try holding on to my private coverage for a few more months until I see how this NHI tax system works(NHI is actually dictatorship at work in a democratic territory)

    This program success depends on good supervision/ management. Thats fair and firm. Who can control and discipline rude and bad attitude staff...As much as i am for NHI.. I'm more scared of the staff than the doctors. My doctor can easily be replace staff can't. Some are so manipulative and lie. They will foxxxxok op the thing

    I'm really tired of hearing people complain. How could you wait until the last minute and still have the audacity to complain about how long you have to wait? Furthermore, I realize that NHI has kinks to iron out and was certainly not ready for operation on January 1st. But have some optimism and understanding. It makes us all healthier when we are not so negative!

    Hon. Minister, I commend your efforts to bring National Health Security to all. I think however for it to work or for the Gov. to be better prepared especially for unanticipated expenses, the Cap of $251.87 needs to be revised. I think it should be removed to collect much more and follow a system similar to the payroll tax system, see below:
